Intervention Presentation and Capstone Video Reflection

Hi, I am Inventory, and today I will be sharing my experience and what I gathered and learned from my capstone project. Throughout my nursing journey, working with patients like Arthur, diagnosed with Type II Diabetes, has been a defining experience. As part of my Capstone project, I had the privilege of applying evidence-based practices to enhance Arthur’s care, addressing the pressing global issue of Diabetes, especially in the US, where millions are affected. Leveraging health technologies like Telehealth and Continuous Glucose Monitoring, we ensured timely interventions and continuous monitoring for Diabetic patients like Arthur  (Carlson et al., 2021). These tools provided invaluable real-time data and remote consultations, breaking down barriers to care, and allowing for effective disease management.

Moreover, my Capstone project embraced the Prosci ADKAR model, which facilitated a structured approach to change management (Angtyan, 2019). By utilizing evidence-based practices and innovative technologies, I not only contributed to Arthur’s improved Diabetes management but also gained invaluable insights into addressing this pervasive healthcare challenge. This experience has been instrumental in my growth as a nurse, reinforcing the importance of a patient-centric, evidence-based approach in providing holistic care and ultimately improving patient outcomes.

Contribution of an Intervention

The NURS FPX 4900 Assessment 5 intervention implemented for Arthur, a patient with Type II Diabetes, has made a substantial contribution to his satisfaction and quality of life. The utilization of Telehealth, Continuous Glucose Monitoring, and the Prosci ADKAR model significantly enhanced his overall experience.

Telehealth played a crucial role in improving Arthur’s satisfaction by providing remote consultations. This not only ensured that he could receive timely medical advice and care but also eliminated the hassle of frequent in-person visits, addressing his mobility limitations and transportation challenges (Shah et al., 2020). He and his family expressed immense relief and gratitude for the convenience Telehealth brought to their lives, improving their satisfaction with the healthcare experience.

Continuous Glucose Monitoring allowed for real-time tracking of Arthur’s blood sugar levels. This not only enhanced the precision of his Diabetes management but also gave him a sense of control over his condition (Skoufalos et al., 2022). By maintaining better glycemic control, he experienced fewer fluctuations in his blood sugar levels, reducing the physical and emotional toll of severe highs and lows. This resulted in a remarkable improvement in his quality of life, as he was better able to engage in daily activities without the constant fear of glucose imbalances.

The Prosci ADKAR model played a significant role in empowering Arthur to embrace lifestyle changes (Angtyan, 2019). By understanding the stages of awareness, desire, knowledge, ability, and reinforcement, he could navigate the process of change effectively. This not only improved his compliance with dietary modifications and medication adherence but also positively impacted his self-efficacy and confidence in managing his Diabetes. He and his family acknowledged the transformation they observed in Arthur’s outlook and habits, appreciating the structured approach that contributed to a sense of empowerment and control over his health.

The intervention, with the integration of Telehealth, Continuous Glucose Monitoring, and the Prosci ADKAR model, not only improved Arthur’s satisfaction and quality of life but also greatly enhanced his family’s experience. The positive feedback received underlines the significant impact these technologies and the change management model had in managing Arthur’s Diabetes and the overall well-being of the patient and his loved ones.

Use of Evidence and Peer-Reviewed Literature Importance of NURS FPX 4900 Assessment 5

The planning and implementation of my capstone project were strongly informed by evidence-based practice, with a focus on self-management and patient education for Diabetes, supported by a review of peer-reviewed literature. Extensive literature and research have consistently demonstrated the benefits of self-management and patient education for individuals with Diabetes. These studies have shown that when patients are actively engaged in their care, informed about their condition, and equipped with self-management skills, they experience improved Diabetes control (Hamidi et al., 2022).

Regarding the healthcare technologies peer-reviewed articles were instrumental in guiding their integration into the project. For example, the utilization of Continuous Glucose Monitoring (CGM) has been substantiated by research showing that it leads to better glycemic control, reducing the frequency of hypoglycemic and hyperglycemic events (Varughese et al., 2020). Similarly, Telehealth backed by literature, has demonstrated its efficacy in improving patient adherence to treatment plans, leading to fewer hospital readmissions and enhanced patient satisfaction (Noel et al., 2020). Studies have further highlighted the role of Telehealth in enhancing the overall accessibility of care for individuals with Diabetes, especially those in underserved or remote areas (Ju, 2020). This technology offers convenient virtual consultations, reducing the barriers to regular follow-ups, ensuring timely interventions, and ultimately contributing to better disease management and patient well-being. The Prosci ADKAR model, informed by evidence-based principles of change management, has been shown to be effective in enhancing patients’ willingness and ability to adopt new habits and adhere to care plans (Banu et al., 2022).

The integration of self-management and patient education, along with the incorporation of healthcare technologies, was informed by a comprehensive review of peer-reviewed literature. This approach ensured that the project’s interventions were aligned with established best practices, promoting the most effective care for patients like Arthur with Diabetes.

Leveraging Health Care Technology

Diabetes is a pervasive and pressing health concern in the United States, affecting millions of individuals and exerting a significant impact on public health (Sulaimon et al., 2019). Its complications are far-reaching and often lead to a cascade of pre-existing medical conditions, such as cardiovascular diseases, neuropathy, and kidney problems. This burden necessitates the urgency of effective Diabetes management and the role of innovative healthcare technologies in addressing these life-altering challenges.

The strategic integration of healthcare technology played a pivotal role in enhancing patient outcomes and communication, with a specific focus on patients like Arthur. The inclusion of Continuous Glucose Monitoring (CGM) allowed for the real-time tracking of blood glucose levels, offering a comprehensive understanding of the patient’s condition and enabling personalized and timely interventions (Varughese et al., 2020). Simultaneously, the adoption of Telehealth platforms proved to be a game-changer, effectively breaking down access barriers, particularly benefiting patients in underserved rural areas (Ju, 2020). Telehealth offers a convenient and efficient channel for patient monitoring and consultations, actively engaging patients in their own care and enabling a collaborative approach to Diabetes management.

As the healthcare landscape continues to evolve, there remains immense potential to further harness the capabilities of healthcare technology in nursing practice. To seize these opportunities, it is crucial to ensure equitable access to these tools, keeping in mind the diverse needs of the patient population. Moreover, nurturing a culture of education and awareness among healthcare professionals is essential to maximize the benefits of these innovations.

In this context, nurses are uniquely positioned to lead the charge in leveraging the Telehealth platform to aid Diabetic patients like Arthur. They can play a central role in facilitating virtual appointments, offering guidance on device usage, and coordinating remote consultations with healthcare providers. By proactively embracing Telehealth, nurses can help bridge the existing gap in Diabetes care, with a particular emphasis on reaching patients in underserved rural areas. Through their expertise and dedication, nurses ensure that patients receive the necessary support, guidance, and resources to effectively manage their condition, ultimately improving their quality of life and reducing the burden of Diabetes-related complications.

Health Policy Influenced the Planning and Implementation of Capstone Project

Health policy played a significant role in influencing the planning and implementation of my capstone project. Policies such as the Affordable Care Act (ACA) have underscored the importance of value-based care and the integration of health technologies into healthcare delivery (Tachinardi & Embi, 2021). These policies have created a framework that encourages the use of Telehealth and Continuous Glucose Monitoring (CGM) to improve patient access to care and enhance chronic disease management, aligning closely with the goals of my capstone project. Additionally, the Food and Drug Administration (FDA) regulations regarding medical devices were critical in ensuring the safety and effectiveness of the technology used in my project (Health, 2023).

In the context of policy development, my capstone project has contributed by demonstrating the practical application and effectiveness of these technologies in managing chronic conditions like Diabetes. The successful integration of Telehealth and CGM into Diabetes care delivery has showcased how these tools can lead to improved outcomes and patient satisfaction (Carlson et al., 2021). These outcomes can serve as valuable evidence in advocating for policy initiatives that promote the broader adoption and reimbursement of these technologies, potentially influencing future healthcare policy changes.

In the role of a baccalaureate-prepared nurse, it is evident that nurses play a critical role in policy implementation and development. Their involvement spans advocating for evidence-based practices, engaging in interdisciplinary collaboration, and communicating the real-world impact of policies on patient care. Baccalaureate-prepared nurses are well-equipped to bridge the gap between policy initiatives and clinical practice, ensuring that policies are effectively translated into patient-centered, high-quality care. Furthermore, they can contribute to policy development by providing valuable insights from their front-line experiences, which can inform future healthcare policy decisions that aim to improve patient outcomes and the overall quality of care (Wichaikhum et al., 2019).

Capstone Project Outcomes of NURS FPX 4900 Assessment 5

The outcomes of my capstone project, aimed at enhancing Diabetes management through the integration of health technologies and evidence-based practices, closely aligned with my initial predictions. The project met and even exceeded my expectations in several key aspects. Continuous Glucose Monitoring (CGM) and Telehealth interventions proved highly effective in improving patient outcomes, with Arthur experiencing better glycemic control, reduced complications, and an improved quality of life. The project demonstrated that these technologies can indeed be adopted as best practices in Diabetes management, providing a scalable and patient-centered approach to care delivery.

One notable achievement was the successful implementation of Telehealth, which broke down access barriers and improved patient engagement. This NURS FPX 4900 Assessment 5 intervention can be easily adopted as a best practice in healthcare settings, particularly for patients in underserved rural areas. The generalizability of this approach is substantial, as it can benefit a wide range of chronic disease management, not limited to Diabetes. The principles of integrating technology, fostering patient engagement, and employing evidence-based practices can be applied to various health conditions, ensuring the generalizability of this intervention beyond the specific setting of my capstone project.

Personal and Professional Growth

Throughout my capstone project, I’ve experienced significant personal and professional growth. As I’ve delved into the complex realm of Diabetes management, my understanding of ethical care and adherence to professional standards has deepened. One of the areas of personal and professional growth that I am most proud of is my ability to provide patient-centered care. By integrating health technologies like Continuous Glucose Monitoring and Telehealth, I have effectively facilitated personalized and evidence-based interventions. This not only improved patient outcomes but also highlighted the importance of actively engaging patients in their care and fostering a collaborative approach. My ethical care provision has been further enhanced through my commitment to ensuring equitable access to healthcare technologies. Recognizing the importance of patient diversity and the potential for technology to bridge gaps, I have worked to make these tools accessible to underserved populations, aligning with the principles of fairness and justice in healthcare. Furthermore, I’ve taken particular satisfaction in my role as a patient advocate. I have actively involved patients and their families in the decision-making process, ensuring that their values and preferences are considered in care planning. This experience has emphasized the importance of autonomy and informed consent in ethical care.

My capstone project has been a journey of immense personal and professional growth. It has strengthened my ability to provide ethical care while adhering to professional standards, particularly in the areas of patient-centered care, equitable access to technology, and patient advocacy. This growth has not only benefited the patients I’ve served but has also enriched my role as a healthcare professional.


The exploration of Diabetes management through the lens of my capstone project has showcased the significant impact of evidence-based practices and healthcare technologies on patient outcomes and quality of care. The integration of Continuous Glucose Monitoring and Telehealth has demonstrated their potential as best practices, breaking down access barriers and promoting patient engagement, while fostering ethical and patient-centered care. Furthermore, my involvement in policy development and advocacy has illustrated the vital role nurses play in translating policy initiatives into clinical practice. The growth in my ability to provide ethical care, focusing on patient diversity and advocacy, has enriched my professional journey, underscoring the transformative power of technology and evidence-based practice in healthcare.
