    Population Health Using a Community Lens

    Population health is concerned with the determinants of social, economic and environmental factors on the health of populations of a given population. When health providers assume a community approach, they can identify and respond to health disparities, learn about those who are at risk, and examine the general environmental influences on the occurrence of disease. It is a new thinking process, which does not emphasize the disease but the health needs and prevention policies within a population. A community-based approach is key for determining health inequities and creating public health strategies that tackle them to enhance the health of the population as a whole (King et al., 2021). The assessment report is devoted to the chosen population of the community of Houston, Texas, and low-income Hispanic adults who are burdened by various health barriers and have a high risk of developing chronic diseases.

    Community of Interest

    Houston, Texas, is a big, culturally diverse metropolitan area of over 2 million people (Understanding Houston, 2025). It is a well-known community for its high ethnic diversity and large Hispanic population, and has great socioeconomic differences at a neighborhood level. Houston was chosen due to this evaluation since it has a diverse demographic makeup and has an extensive health disparity that can be used to explore the issue of population health among vulnerable groups. Several industries facilitate the economy of the city, such as healthcare, energy, manufacturing, and transportation. Even with this economic strength, there are still huge disparities in health in Houston, including between the rich and the poor. Some communities have limited access to health care services, are at risk for food insecurity and transportation issues, and have less access to preventive health education. These kinds of conditions contribute to the establishment of disparities in health between groups. Income, education, housing, and access to health care services are considered social determinants of health that have a significant influence on health outcomes (Vrtikapa et al., 2025). Thus, Houston is an appropriate place where the burden of chronic diseases among at-risk populations should be investigated.

    At-Risk Population

    The population targeted for being at-risk is low-income Hispanic adults in Houston, TX. This population is a large proportion of the population in the city, and it is disproportionately affected by factors, including socioeconomic and structural barriers that have a major impact on health outcomes. This population was chosen due to its being one of the most vulnerable and high-risk populations in Houston, especially with regard to the prevalence of chronic diseases and low access to healthcare services. These individuals have health care limitations, and most of them lack insurance coverage, face language barriers, have transportation issues, and have financial problems. The results of such barriers can be a delay in diagnosis, improper management of the disease, and a decrease in preventive services. Socially and economically disadvantaged people are far more prone to developing chronic illnesses and poor health outcomes in the long-term (Livingston et al., 2025). Other obstacles to effective communication with health care providers and involvement with prevention services could be associated with cultural and linguistic resources.

    Health Problems

    Diabetes type 2, overweight, and hypertension are major health problems among low-income Hispanic adults in Houston, Texas. In Harris County (including Houston), the prevalence of diabetes among adults is approximately 13.5%, which means that there is a high rate of chronic diseases within the population in the area (Vision Wrights, 2026). The prevalence of diabetes is particularly high in underserved and low-income populations, where there is limited access to preventive care and disease management resources.

    The obesity rate in the area is also extremely high, as 71.8 of all adults in Harris County are estimated to be obese, according to the CDC PLACES (Cities for Better Health, 2014). Local datasets indicate that the prevalence of obesity is almost 37 per cent in the county, especially among the minority and the low-income groups, where food insecurity and accessibility to safe physical activity spaces are prevalent (Vision Wrights, 2026). Another significant issue is hypertension, as about 32.6% of adults in Harris County have high blood pressure, and the control rates are lower than in underserved communities because of gaps in primary care access and preventive screening (Vision Wrights, 2026).

    The socioeconomic conditions and lifestyle have continued to be leading causes of these health conditions. Food insecurity (about 39% of households) and high neighborhood-level socioeconomic inequalities also contribute to increased risk of chronic diseases in Harris County (Ellis, 2025). Poor access to affordable healthy food decreases opportunities to engage in physical activity, physical access to transportation, and healthcare access restrictions all contribute to disease development and evolution. Language barrier and financial limitations further postpone diagnosis and inadequate compliance with treatment regimens. These disparities in the prevalence and severity of chronic illnesses in vulnerable groups remain predominantly influenced by social factors of health (Vrtikapa et al., 2025).

    Health Problem Patterns

    The incidence of diabetes, obesity, and hypertension in Texas reveals trends that are at least equal to those nationwide and reveals the high burden of chronic disease in the state. The prevalence of diabetes among adults in Texas is approximately 12.3% as compared to 11.6% among adults in the entire country, which suggests a higher prevalence rate in the state (American Diabetes Association, 2023). Likewise, the prevalence of obesity is a significant community issue, with about 35.0% of adult Texans considered to be obese compared to the national average of 32.8 (America’s Health Rankings, 2026). A large percentage of the adult population is also at risk of hypertension. According to recent estimates, about 34.7% of adults in Texas are hypertensive, which is in contrast to 32.6% of the nation (America’s Health Rankings, 2025). In comparison with other states, like Colorado, with the prevalence of obesity being about 25.1% and diabetes being about 8.3%, Texas has a significantly greater burden of chronic disease (America’s Health Rankings, 2026). These comparative figures prove that Texas regularly having more rates of diabetes, obesity, and hypertension as compared to most other states, and, on numerous occasions, it is well above the national averages. The increased incidence of the diseases highlights the necessity of special prevention and management measures, especially in vulnerable populations that have other socioeconomic and healthcare access challenges.

    Addressing the Health Problems

    Most international health bodies, such as the World Health Organization (WHO), are very concerned with chronic diseases such as diabetes, hypertension, and obesity. WHO suggests international strategies to decrease noncommunicable disease (NCD) (improved nutrition, increased physical activity, prevention of tobacco use, and better access to health services). One of these priorities is the WHO Global Action Plan on the Prevention and Control of Noncommunicable Diseases, which aims at reducing the number of premature deaths due to chronic disease through the reduction of the main modifiable behavioural and environmental risk factors. The global health strategies also include chronic disease control (Thomas et al., 2023), which is a significant consideration of prevention, early detection, and equal access to health care services. These can be applied at the local level in cities such as Houston to enhance the population health outcomes of vulnerable populations.

    Population Vulnerability

    Since a number of social and economic factors overlap, and access to health care is also problematic, low-income Hispanic adults in Houston can be regarded as vulnerable. These may be poverty, lack of health insurance, language barrier, lack of health literacy, and inability to receive preventive health care. These risk factors contribute greatly to the risk of developing chronic diseases and make it more difficult to effectively manage those that already exist.

    Diseases affect the vulnerable populations disproportionately since there are systemic inequalities, and at the same time, there are no available basic health services in such societies. Another key social determinant of health that can significantly impact the health of this population is better access to health care. This is possible through increasing accessibility of services through community clinics, health promotion screening, culturally responsive care, interpreter services, and improving early disease detection and management. By placing emphasis on these aspects, one can reduce the incidence and effect of diabetes, obesity, and hypertension and improve health and wellness in the long term.
