Mental health has become an increasingly important issue in the public health arena due to the diminished burden of depression, anxiety, cognitive impairment, and social isolation in older adults with population aging. In Orlando, Florida, many older adults aged 65 years and above experience diverse issues such as the absence of geriatric mental health care, economic instability, and immobility. These issues are extremely interrelated with social determinants of health (SDoH) and health disparities (Katarina Vrtikapa et al., 2025). Healthcare programs have increased, and many still focus on mental disease rather than on social determinants of mental illness. This paper aims to explain the effect of an upstream policy on the social determinants of health of the elderly population that face mental health issues in Orlando and the role of ethics in policymaking.

Seniors and Mental Health

The elderly population who are 65 years and above is vulnerable since it is characterized by chronic diseases, loss of a loved one, isolation in society, and lack of financial resources. These are the factors that help advance the risk of depression, anxiety disorders, and a reduction in the quality of life (Yazdi-Feyzabadi et al., 2025). Transportation challenges affect the majority of seniors in Orlando, making them unable to access health care and community services, and prohibit their ability to access in addition to the inability to access family incomes that lack disposable income (Alignment Health, 2024). All these issues underscore essential social determinants of health (SDOH) such as economic stability, access to health, and social connectedness. The inability to fulfill them leads to the likelihood of mental health problems, and it influences healthy aging (Zhuo et al., 2025). Community support and preventive services are important in the translation of these social determinants into improved population health and fewer disparities among older adults.

Upstream Policy Clinching Social Determinants of Health

The Older Americans Act (OAA) is an upstream policy that looks at the upstream causes of poor health instead of just treatment (Colello & Napili, 2025). The act assists in maintaining the independence and connectedness of older people by developing transportation and nutrition programs, supporting caregivers, promoting health, and community engagement. These services have a direct contribution to the health determinants like transportation, food security, economic stability, and social support. In contrast to downstream approaches, which have predominantly been concerned with treating mental health problems. The OAA is a proactive strategy that aims to reduce risk factors until they develop into severe mental health problems (Katarina Vrtikapa et al., 2025). The policy promotes community-level investment in services and boosts health equity and minimizes disparities in mental health and well-being among the elderly.

The intervention is useful to the mental health of older adults as it will give them increased access to counselling services and social interaction, and will ensure the older adults do not feel lonely. The possibility of forming social relationships, which have been found to decrease depression and anxiety, is given to older adults by community-based support groups, such as in the case of younger adults. Likewise, frequent screening for mental health in primary care can help detect early signs of mental illness and offer prompt intervention, resulting in better psychological health and overall quality of life.

Impacts of the Policy

Orlando has numerous older adults today who now enjoy improved access to transportation, nutrition services, behavioral health referrals, and community wellness programs due to the improvements in the Older Americans Act. The services will encourage early detection of depression and anxiety, reduce feelings of loneliness, and enhance the quality of life (Huang et al., 2026). Preventive measures also contribute to reducing unnecessary hospitalization and cost-saving in the long run for population health. Through this intervention, it has been possible to improve the accessibility of early assessment and timely treatment and subsequent support services to support older individuals with regard to their mental health. Regular check-ups of patients by health workers will help in the identification of depression, anxiety, and/or cognitive impairment signs in an individual, and a referral will be made to get the required care promptly to ensure that the patient becomes less depressed, anxious, and/or cognitively impaired. There are also community-based, counselling, and social support groups available to aid in reducing loneliness, enhancing social bonding, and making a voice heard by an individual in their daily life. These evidence-based interventions are based on the needs that allow emotionally well-being by catering to the physical, psychological, and social requirements that create a better quality of life and a solution to healthy aging. Funding, staffing, and access to services are different in communities, and the policy is not effective. These issues encourage the fact that further investment is necessary to make sure that mental health services can be accessible to all older adults.

Role of Ethics in Policymaking

The focus of population-health policies ethics is that it is applicable to uphold beneficence, justice, autonomy, and nonmaleficence. These principles are embodied in the Older Americans Act, which directs resources to the most vulnerable older adults who are economically insecure, disabled, and/or socially isolated.
