Blog: Understanding Healthcare Payment Models and Their Impact

The healthcare system in the United States is still in a state of constant change and is looking for the best methods to fund care and to deliver the best possible outcomes for patients. The payment models have become an integral part of patient-centered, accessible, and experienced care delivery. As a healthcare worker, especially a nurse, it is crucial to understand these models to add to quality and safety in care.

Common Healthcare Payment Models

1. A commonly used model is the Fee-for-Service (FFS) payment system, where the provider receives payment for each service rendered. The model is known to result in increased health care utilization, and it has been criticized for rewarding “volume” not “quality” of care (Etges et al., 2023). However, despite its limitations, FFS has become one of the most prevalent models in the U.S. and is a model that has influenced the delivery of care by many organizations.
2. The second is the Value-Based Care (VBC) model, which is based on payments being tied to patient outcomes. This model incentivizes providers to achieve efficiency, quality, and better health outcomes for their patients without being tied to a specific volume of procedures performed (Leao et al., 2023). This represents a move towards focusing on patient-centred outcomes and improving health long-term.
3. A third option is the Capitation system, in which providers are paid a fixed amount per patient for a specific period, irrespective of the number of services the patient utilizes. While capitation promotes the prevention of health problems and the control of costs, it can at times result in issues of suboptimal utilization of needed preventive services (Morenz et al., 2023). Capitation can be used appropriately to achieve cost savings and proactive population health management.

Impact of Value-Based Care on Patients

It centers on the Value-Based Care model and has a strong impact on a variety of dimensions. Providers implement evidence-informed practices and care teams to facilitate coordination in the delivery. Access is a focus of VBC efforts, and frequently the programs increase the availability of preventive care. Value-based models establish a new financial incentive structure that can lead to more equitable, high-quality care. But being successful requires good data collection and infrastructure to fairly measure the outcomes of the various populations.

Needed Changes in U.S. Payment Models

Several changes within the U.S. health system are needed to empower nurses and to minimize disparities. First, it is important to include more nursing-sensitive indicators in the payment models so that more direct effects of nursing care are made visible in patient outcomes. Second, nurse-led clinics and community-based interventions should be reimbursable to make it more accessible for underserved populations. While NP and APNs are well suited to provide primary and preventive care in a resource-limited community, the payment systems can limit this potential.

Last but not least, the system needs to increase investments in health equity programs in payment models. Including equity measures would encourage organizations to take steps to reduce inequities in the provision of health services and in social determinants of health. Such a shift would enhance quality and safety as well as raise the nursing profession’s profile in leading the way for system change.
