Practicum Goals

Goal 1: Develop and execute personal diabetes care plans based on the preferences, goals and lifestyle of the patient along with interdisciplinary teams including dietitians, physical therapists, and social workers.

Goal 2: Incorporate ethical and safe care transition/discharge planning with diabetic patients with emphasis on continuity of care, medication safety and support services within the community.

Goal 3: To complete 10 or more applicable clinical hours per day to meet the 40 practicum hours that I must complete in this course, as well as assessing the outcomes of care coordination on the basis of the glycemic control, patient satisfaction, and readmission rates.

Notes: My preceptor will oversee and ensure that my daily clinical activities are as per course requirements and they are confirmed. These will assist with the real development of skills and must be done prior to the submission of Assessment 2.

Practicum Schedule

Notes: Every week I will attend diabetes education rounds, interdisciplinary team care planning and community outreach in relation to management of chronic diseases. The strategy should include completion of 10 hours a day, which is to be monitored by my preceptor.

Action Item: Submit weekly reflection logs, at least three coordinated care intervention based on the case documentation of diabetic patients, and fulfill the 40-hour requirement, not more than 12 hours a day.

Practicum Documentation

Notes: In its attempt to realize such a practice, documentation will include EHR records, team communication notes, and patient feedback logs in an effort to ensure continuity and quality of care. All the clinical activities are to be duly documented with one hour of the time being devoted to the conference call. The maximum number of logged hours is 12 hours per day and 10 clinical hours are required prior to the submission of Assessment 2. The entire documentation procedure will also have to complete the course evaluation and submit feedback at the end of the practicum.

Action Item: Ensure that all entries are HIPAA compliant and that there is timely and accurate care transition especially during discharge and follow-up care.

Expectations

Notes: I will also have to participate in interdisciplinary meetings actively, apply evidence-based measures to structure diabetes care, and consider the use of digital technology, such as the utilization of mobile health applications in patient engagement.

Action Item: I will provide a final evaluation form that will be signed by my preceptor and reflect on the end of practicum concerning the challenges I encountered and the objectives that I achieved in the area of diabetes care coordination.

Summary

The domain in which I was interested in is holistic, ethical, and data-driven coordinated care to manage diabetes. Group-based interventions, community resources, and digital technologies to enhance chronic care management were also used by me.

The call entailed the verification of clinical hours, SMART goals, paperwork, the course evaluation and feedback procedure, as well as a series of communication vehicles with the course instructor via email.
