Practicum Goals

Goal 1: Develop and execute personalized diabetes care plans based on the patient’s preferences, goals, and lifestyle, in collaboration with interdisciplinary teams that include dietitians, physical therapists, and social workers.

Notes: The Preceptor will oversee and ensure that my daily clinical activities are in accordance with course requirements and are confirmed. These will assist with the real development of skills and must be done before the submission of Assessment 2.

Practicum Schedule

Notes: Every week, I will attend diabetes education rounds, interdisciplinary team care planning sessions, and community outreach events related to the management of chronic diseases. The strategy should include completing 10 hours a day, which will be monitored by the Preceptor.

Action Item: Submit weekly reflection logs, at least three coordinated care interventions based on the case documentation of diabetic patients, and fulfill the 40-hour requirement, not more than 12 hours a day.

Practicum Documentation

Notes: To implement this practice, documentation will include EHR records, team communication notes, and patient feedback logs, ensuring continuity and quality of care. All clinical activities must be duly documented, with one hour allocated for the conference call. The maximum number of logged hours is 12 hours per day, and a minimum of 10 clinical hours is required before submitting Assessment 2. The entire documentation procedure will also require completing the course evaluation and submitting feedback at the end of the practicum.

Action Item: Ensure that all entries are HIPAA compliant and that there is timely and accurate care transition, especially during discharge and follow-up care.

Expectations

Notes: I will also need to actively participate in interdisciplinary meetings, apply evidence-based measures to structure diabetes care, and consider the use of digital technology, such as mobile health applications, for patient engagement.

Action Item: I will provide a final evaluation form that will be signed by the Preceptor, reflecting on the challenges I encountered and the objectives I achieved in the area of diabetes care coordination at the end of the practicum.

Summary

The domain in which I am interested is holistic, ethical, and data-driven coordinated care to manage diabetes. I also utilized group-based interventions, community resources, and digital technologies to enhance chronic care management.
