NURS FPX 6200 Assessment 5 Practicum Hours

During the 50 hours of my practice, I participated in an active strategy of quality improvement that aimed to improve patient discharge education, decrease the 30-day readmission rates, and enhance interdisciplinary teamwork in an inpatient hospital facility. I started with a SOAR (Strengths, Opportunities, Aspirations, and Results) analysis to evaluate the existing structure of the organization, safety practices and leadership model. This also involved participation in interdisciplinary safety huddles, review of discharge procedures, and analysis of patient satisfaction data to determine areas that need improvement.

To learn about the workflow of discharge planning and Electronic Health Record (EHR) integration, I spent a large part of my time working with nurse managers, case managers, pharmacists, and IT people. I also witnessed the teach-back education methods in the discharge process, identified the obstacles, including limited time and language barriers, and assisted in creating the plans to enhance staff involvement. I also attended stakeholder meetings and provided data-based outcomes and took part in planning sessions, which are designed to develop culturally competent discharge tools and onboarding procedures for Transitional Care Nurses (TCNs).

I also engaged in the creation of training modules on cultural humility, teach-back communication, and the documentation process among staff. I examined patient education materials and helped to translate these materials into the five most commonly spoken patient languages in the facility. Such practice hours were also accompanied by simulation activities, orientation feedback analysis, and reviews on quality metrics using performance dashboards. The 50 hours was an extensive, practical experience in implementing leadership, communication, and systems-thinking skills in a clinical environment that is aimed at enhancing patient outcomes and safety.
