The Honorable ________ Office of the Governor California State Capitol Sacramento, CA, USA Dear Governor __________, California’s primary care doctors are reporting a common problem: staffing shortages. Crowded clinics, diminished access to high-quality health care, and longer waits for diagnosis are common consequences of a shortage of health professionals, especially in rural areas and areas unserved by government-supported facilities. These shortages also place additional strain on existing clinical teams, reducing overall care efficiency. Executive leadership can play a key role in overcoming these challenges through policies that increase the workforce, attract specific funding streams, enhance the infrastructure that supports safety-net providers, and support professional retention. California’s overburdened and understaffed health service sector is struggling to accommodate an increasing volume of patients, with doctors spending less time with each patient, and the delivery of quality care is adversely impacted. This impacts communities in low-income, minority, and rural areas that still experience significant disparities in access and health outcomes. Simultaneously, increasing workloads for health-care professionals have been associated with more medical errors, adverse outcomes, and burnout and turnover of hospital employees (Aiken et al., 2023). This, in turn, coupled with rising costs, is forcing providers to explore new ways to sustain services without increasing costs. Policy initiatives such as loan forgiveness and financial incentives for physicians could be a solution, particularly in rural and underserved areas. These programs can provide incentives for staffing, as well as ensuring workforce retention in “hard-to-serve” locations. While government incentives involve a short-term cost, they can lead to savings in future staffing costs and enhance workforce stability. These approaches can boost workforce stability and productivity, leading to a more efficient and quality-focused health care system in California. I have been impacted personally and professionally by understaffing issues in many primary health care settings as a health care provider. In a recent clinical rotation, I witnessed a particularly challenging environment in which patients waited considerable time for consultations, with clinicians forced to see many patients in a short time. The time pressure resulted in quicker clinical decision-making and shorter time for investigation, ultimately leading to delays and quality-of-care issues. This incident underscored the need for appropriate resource allocation and risk mitigation strategies, such as optimal staffing levels, staff support initiatives, and good operational planning, to ensure safe and efficient delivery of services. Given these challenges, I would encourage consideration of legal and financial incentives that promote safe staffing ratios, along with increasing undergraduate and graduate education and training programs and recruitment and retention incentives in primary care. These efforts would not only achieve better health outcomes but also help overcome longstanding health disparities and promote the sustainability of the California healthcare system. I would also call for continued investments of available resources in programs that address the issues facing health care staff, so we have more sustainable and equitable health care services for our community.
